Visit Reason
The Office of the State Fire Marshal conducted a fire safety inspection at the Aegis Senior Inn of Redmond facility to assess compliance with fire protection and life safety codes.
Findings
The inspection found multiple violations including fire doors that would not close and latch properly, lack of documentation for required annual inspections and tests of fire resistance materials, sprinkler systems, fire extinguishers, fire alarm systems, and fire department connections. Additional issues included obstructions around fire department connections, improper emergency egress obstructions, and a delayed egress door that did not open within the required time.

Description
The gas appliances on casters in the kitchen are not limited by a restraining device.
Facility is unable to provide documentation that the annual fire resistance rated construction material inspection has been completed.
Resident room 309 fire door would not close and latch from the fully open position.
Resident room 219 fire door would not close and latch from the fully open position.
The fire rated door from the memory care dining room to the corridor would not close and latch from a fully open position.
Resident room 137 fire door would not close and latch from the fully open position.
The double fire doors to the main dining room were installed with an astragal and lacked a door coordinator to ensure they close and latch properly.
Facility is unable to provide documentation for the annual sprinkler system inspection.
Facility is unable to provide documentation for the 3 year dry system full flow trip test.
Facility is unable to provide documentation for the annual dry system trip test.
Facility is unable to provide documentation for the annual forward flow test in accordance with NFPA 25.
The protective covering on the water flow switch in the riser room has been removed.
The required annual maintenance for the fire extinguisher in the elevator equipment room has not been completed.
Facility is unable to provide documentation for the annual fire alarm system testing.
There is shrubbery obstructing the working clearance of 36 inches around the fire department connection.
Facility was unable to provide documentation that the Fire Department Connection has been hydrostatically tested in accordance with NFPA 25.
Retractable 'do not enter' ropes were installed blocking emergency exits in the 2nd floor stairwell near room 202 and in front of the emergency exit door in the living room.
The emergency egress light near room 318 would not illuminate when the test button was pressed.
The delayed egress door located at the main entry of the memory care unit did not open within 15 seconds.
