Description
A. A manager shall ensure that: 7. Documentation is maintained for at least 12 months after the last date on the documentation of the caregivers and assistant caregivers working each day, including the hours worked by each; - Failed to maintain documentation of caregivers and assistant caregivers working each day, including hours worked.
C. A manager shall ensure that a personnel record for each employee or volunteer: 1. Includes: c. Documentation of: ix. Documentation of compliance with the requirements in A.R.S. § 36-411(A) and (C); - Failed to include documentation of compliance with A.R.S. § 36-411(C)(1) for two of three personnel sampled.
A. Except as required in subsection (B), a manager shall ensure that a resident has a written service plan that: 4. Is reviewed and updated based on changes in the requirements in subsections (A)(3)(a) through (f): b. As follows: iii. At least once every three months for a resident receiving directed care services; and - Failed to review and update a resident's written service plan at least once every three months.
A. A manager shall ensure that: 4. A disaster drill for employees is conducted on each shift at least once every three months and documented; - Failed to conduct and document employee disaster drills on each shift at least once every three months.
B. A manager: 3. Except as provided in subsection (A)(6), designates, in writing, a caregiver who is: a. At least 21 years of age, and b. Present on the assisted living facility's premises and accountable for the assisted living facility when the manager is not present on the assisted living facility premises. - Failed to designate in writing a caregiver present and accountable when the manager was not present.
F. When medication is stored by an assisted living facility, a manager shall ensure that: 1. Medication is stored in a separate locked room, closet, cabinet, or self-contained unit used only for medication storage; - Failed to ensure medications were stored in a locked room, closet, cabinet, or self-contained unit.
A. A manager shall ensure that: 9. Soiled linen and soiled clothing stored by the assisted living facility are maintained separate from clean linen and clothing and stored in closed containers away from food storage, kitchen, and dining areas; - Failed to ensure soiled linen was stored in a closed container, with soiled linen observed on the laundry room floor.
