Visit Reason
The purpose of this visit was to investigate intake #GA50004248, which involved an onsite visit beginning on 7/30/2025 and ending on 8/7/2025.

Complaint Details
The investigation was initiated due to intake #GA50004248 concerning elopement incidents involving Resident #1. The resident eloped twice (5/16/2025 and 7/3/2025), was found uninjured, but the facility failed to report these incidents timely to the Department as required.
Findings
The facility failed to properly maintain the exterior of the home, including damaged fences and unsecured gates, creating unsafe conditions for residents at risk of elopement. Additionally, the facility failed to report to the Department the initiation and discontinuation of Mattie's call within 30 minutes of communication with local law enforcement regarding elopement incidents of a resident.
