Visit Reason
The Minnesota Department of Health conducted a full survey to evaluate and assess compliance with state licensing statutes for Amerigrace Home Care LLC.
Findings
The licensee was found in substantial compliance but had multiple deficiencies including issues with meal contract requirements, emergency preparedness plan, fire safety code violations, physical environment maintenance, fire safety and evacuation plans, and medication storage security.
