Visit Reason
The Minnesota Department of Health conducted a full survey to evaluate and assess compliance with state licensing statutes for Applewood Home Health Care LLC.
Findings
The licensee was found in substantial compliance but had several level two widespread violations including failure to prepare and serve food according to Minnesota Food Code, lack of a complete emergency preparedness plan, inadequate fire safety and evacuation plans and training, failure to maintain and interconnect smoke alarms, improper medication refrigerator temperature monitoring, and failure to post required electronic monitoring signage.
