Visit Reason
The inspection was conducted to assess compliance with regulatory standards related to the facility's provision of a homelike environment during meals and the proper handling and administration of medications, including expired insulin.

Findings
The facility failed to provide a homelike dining environment as residents were served meals on trays and drinks in cartons rather than on tables with proper dinnerware. Additionally, the facility administered expired Lantus insulin to one resident and had expired insulin vials on medication carts, indicating lapses in medication management.

Deficiencies (3)
Residents were served lunch meals on trays on tables and milk, healthy shakes, and nutritional supplements were served in cartons and plastic containers rather than in a homelike manner.
Resident #39 was administered expired Lantus insulin on 3/19/18 and 3/20/18.
Outdated insulin vials were found on a medication cart, including vials labeled with expiration dates exceeding the 28-day use period after opening.
