Visit Reason
Facility failed to ensure service plans reflected current resident needs and provided clear instructions to staff for multiple residents. Medication and treatment orders were not carried out as prescribed, resulting in unnecessary pain and emergency visit. Facility failed to notify physician when residents refused treatments. Staffing orientation and training requirements were not met. Fire and life safety training for residents was not provided within 24 hours of admission.
