Inspection Reports for Avery Garden Assisted Living

An unannounced annual inspection was conducted by Licensing Program Analyst Yolanda Delgado to evaluate compliance with state regulations for the assisted living facility.

The facility was found to be in compliance with all documentation, physical plant, safety, food service, and employee record requirements. No deficiencies were cited during this inspection.

The visit was an announced pre-licensing inspection conducted to evaluate the facility for Residential Care Facility for the Elderly licensure with a capacity of six residents.

The facility was found to be clean, in good repair, hazard-free, and met all operational requirements for licensure. No deficiencies were identified during the inspection.

The visit was conducted as an initial licensing evaluation for Avery Garden Assisted Living-Riverside Facility to assess the applicant/administrator's understanding of community care facility licensing laws and readiness for operation.

The applicant/administrator participated in a telephone interview confirming knowledge of licensing laws, facility operation, admission policies, staffing, health conditions, emergency preparedness, complaints and reporting, and pre-licensing readiness. Signed documentation and photo ID were obtained.
