Individual Service Plans (ISP) were not current, accurate, or available for review for several residents.

Oxygen cylinder tanks were not stored securely, and 'Oxygen in Use' signs were missing in resident rooms using oxygen therapy.

Gas heating system was not properly maintained or inspected annually by qualified personnel.
Fire extinguishers were not inspected yearly or serviced as required.
