Visit Reason
The inspection was a full onsite survey conducted to assess compliance with the state requirements of 7 NMAC 8.2, Regulations for Assisted Living Facilities.
Findings
The survey identified deficiencies related to staff qualifications, fingerprint and criminal history screening, medication administration, custodial drug permits, nutrition, and food safety. Deficiencies could potentially affect the safety and welfare of residents.

Description
Facility failed to ensure all direct care staff had documentation for fingerprints submitted to the CCHSP within 20 days of hire and were cleared by the Employee Abuse Registry prior to start date.
Medication Administration Records (MARs) and physician orders for 4 residents were incomplete, inaccurate, or missing required information.
Facility failed to maintain proper custodial drug permits and comply with medication storage, labeling, and documentation requirements.
Oxygen bottles were stored in residents' rooms without proper signage posted in the facility and on the doors of storage areas.
Nutrition services failed to provide planned and nutritionally balanced meals according to dietary guidelines and failed to maintain proper food safety and sanitation practices.
Facility failed to maintain daily/monthly logs for refrigerator and freezer temperatures and dishwasher cleanliness and functionality.
