The investigation was complaint-driven based on multiple intakes. The incident involved Resident #1 who fell outdoors while being monitored by staff. The resident sustained a head injury, was transported to a hospital, and later died. Staff interviews indicated a failure in supervision during the incident.
Findings
The assisted living community failed to provide adequate and appropriate care and services to one resident, resulting in a fall incident where Resident #1 was found face down with a head injury. Resident #1 was transported to a hospital and later died of pneumonia and COPD. Staff interviews and record reviews revealed lapses in monitoring during the incident.
