Visit Reason
The inspection was conducted as a renewal inspection of the Birchwood at Hillcrest facility.
Findings
The inspection identified deficiencies related to food service, housekeeping, and physical plant conditions, including refrigerator temperature issues, uncovered garbage containers, and dirty kitchen floors and mats with food debris.
Deficiencies (3)
Refrigerator temperature was 44 degrees; freezer had no thermometer in place.
None of the garbage containers in the kitchen had lids on them.
The kitchen floor and mats were dirty with accumulation of dirt and food debris under sinks and kitchen appliances.
