The inspection was a Full-Onsite annual survey conducted for compliance with state regulations for Assisted Livings for Adults.
Findings
Multiple deficiencies were cited related to individual service plans, nutrition, kitchen sanitation, maintenance of building and grounds, lighting, fire safety, hospice care, memory care, and staff training. Plans of correction were provided for each deficiency.

Nutrition services failed to meet requirements including meal service policies, kitchen sanitation, and food storage.
Maintenance of building and grounds was deficient with trash and debris present in outdoor areas.
Kitchen utensils were not stored in a clean, dry place protected from contamination.
Lighting and lighting fixtures were not maintained; emergency lights failed to operate properly.
Windows had damaged or missing screens, exposing residents to insects and debris.
Fire extinguishers were not inspected monthly and lacked proper tags for recent months.
Hospice care training and documentation were deficient; staff had not completed required training hours.
Memory care unit residents lacked appropriate secured environment and documentation.
