Visit Reason
The inspection was conducted as a Renewal Licensing Study to evaluate compliance with licensing rules and regulations for the facility.

Findings
The facility was found to be in non-compliance with multiple rules including tuberculosis screening for residents and employees, meal census record keeping, proper waste container maintenance, kitchen cleanliness, food labeling, and safe storage of hazardous materials. Violations were established for each of these areas.

One resident's tuberculosis screening record could not be located.
Four employees had tuberculosis screening completed the same day as occupational exposure and one employee had no TB test record.
No record of a meal census for the preceding 3-month period was maintained.
Multiple garbage or rubbish containers lacked tight-fitting lids in common areas, laundry, kitchenettes, and main kitchen.
Memory care unit kitchenette equipment and work surfaces were not maintained in a clean and sanitary condition.
Multiple food items were found unlabeled in the memory care area kitchenette refrigerator, employee lounge, and main kitchen.
Hazardous and toxic chemicals were stored in unlocked cabinets accessible to residents in assisted living and memory care areas.

Findings
The facility was found to be in non-compliance with multiple rules including lack of tuberculosis screening evidence for one resident, failure to maintain meal census records, inadequate laundry and linen storage, unlabeled food items, incomplete dishwasher sanitization records, improper use of mechanical closets for storage, and unsafe storage of hazardous materials.
