Visit Reason
The re-licensure survey identified 14 deficiencies including failure to report injuries of unknown cause as suspected abuse, incomplete resident move-in evaluations, inadequate service plans, failure to monitor changes of condition, inaccurate medication administration records, incomplete implementation of Acuity-Based Staffing Tool, incomplete staff training, and fire and life safety deficiencies. Several deficiencies were corrected by the revisit date 2025-01-16, while some remained uncorrected.
