The facility underwent eight inspections between August 1, 2021 and July 31, 2025, resulting in four violations and four inspections with no violations. The inspections spanned a variety of survey types, including relicensure, complaint, and other surveys, and covered a range of regulatory areas.
The inspection with the highest number of violations was the Complaint Survey issued on August 28, 2024, which identified one violation. The cited regulation was 488.4 (c) (9), addressing admission and retention standards.
