Inspection Report Summary for Byron Park

Byron Park, a 151‑bed assisted‑living facility in Walnut Creek, has undergone a series of unannounced licensing evaluations by the California Department of Social Services since late 2022. The initial pre‑licensing inspection on 27 December 2022 identified a deficiency with fire extinguishers that did not meet California’s safety standards. The agency issued a citation, and the problem was later remedied in a follow‑up visit on 4 January 2023, when the LPA cleared the citation and noted no further deficiencies. The pre‑licensing process was completed without additional issues, allowing the facility to obtain its operating license.

In April 2023, a post‑licensing inspection uncovered two separate compliance gaps. One cited the hot‑water temperature in the Bistro, measured at 118 °F, which exceeds the state‑mandated limit. The other highlighted that the facility had not transferred or verified criminal‑record clearances for 3 of 72 staff members, a requirement under Health and Safety Code section 87355(e)(3). The agency issued a plan of correction with a due date of 24 April 2023 and noted that failure to comply could result in a civil penalty. A follow‑up visit on 18 April 2023 found that these corrective actions had been taken, and no further citations were issued.

Subsequent case‑management visits in July and September addressed two separate resident‑related incidents—a medication error in June and an unusual incident involving residents R1 and W1 in September. Both visits involved interviews with senior staff, review of protocols, and retraining sessions, but resulted in no new citations. Throughout the review period, the facility maintained adequate staffing, safe environmental conditions, and compliance with licensing regulations, with all cited deficiencies corrected promptly and no recurrent violations reported.
