Visit Reason
The Adult Care Licensure Section conducted a follow-up and annual survey on 07/22/25-07/24/25 with an exit conference via telephone on 07/24/25.

Findings
The facility failed to ensure exit doors had audible alarms during daytime hours for a resident with dementia, failed to protect food from contamination, failed to serve therapeutic diets as ordered, failed to administer medications as ordered for two residents, and failed to have a physician's order for a resident to self-administer a topical medication.

The Adult Care Licensure Section conducted an annual survey of Calyx Living of Durham on February 21-22, 2023 to assess compliance with regulations related to nutrition, food service, and medication administration.

Findings
The facility failed to have a therapeutic diet menu for one resident with a physician-ordered no-concentrated sweets (NCS) diet, and failed to ensure medications were administered as ordered for two residents, related to an inhaler and medication for inflamed gums.
