CAREFIELD PLEASANTON, a 740‑type residential care facility, received a series of unannounced inspections and case‑management visits between July 2022 and August 2023. In its July 14, 2022 visit, a licensing program analyst (LPA) conducted an infection‑control inspection and found the facility in compliance with COVID‑19 mitigation and hand‑washing protocols, reporting no deficiencies. Two weeks later, on July 28, the LPA reviewed a death report for resident R1, who passed on July 22 after a severe cerebral infarction and chronic conditions. The review confirmed that the resident’s medical and DNR status were properly documented, and the visit concluded with no deficiencies cited.
A priority‑2 health‑and‑safety inspection on December 30, 2022, confirmed that the facility maintained proper temperature control, adequate food and medication storage, functional fire protection equipment, and a secure medication room. The report noted no deficiencies and reaffirmed the facility’s compliance with state regulations. In contrast, the August 29, 2023 required‑one‑year inspection revealed several regulatory gaps. The LPA documented missing tuberculosis test results for residents R2 and R3, a lack of current first‑aid training for staff member S4, and an initially high hot‑water temperature of 126 °F in a resident’s bathroom (later corrected to 107.5 °F). The inspection generated a formal deficiency notice with a due date of September 13, 2023, and a plan of correction requiring submission of TB test results and updated first‑aid certification. The water‑temperature issue was promptly rectified during the visit, and no further deficiencies were identified thereafter.
