Visit Reason
The inspection was conducted as a Renewal Licensing Study to evaluate compliance with licensing requirements and to determine if the facility meets standards for license renewal.
Findings
The facility was found to be in non-compliance with several rules including failure to notify the department of administrative changes, incomplete narcotic count logs, insufficient medication order instructions, lack of meal census records, and incomplete kitchen sanitation logs. A repeat violation was noted for kitchen and dietary sanitation recordkeeping.

Visit Reason
The inspection was conducted as part of the original licensing study for Charter Senior Living of Linden to determine compliance with applicable licensing statutes and administrative rules for a home for the aged.
Findings
The facility was found to be in substantial compliance with licensing requirements after an on-site inspection identified a few physical plant deficiencies which were corrected. The Health Facilities Engineering Section approved a total capacity of 104 beds, and fire safety certification was issued.
