Follow-up survey conducted to determine correction of orders found on the survey completed on November 27, 2023.
Findings
The facility had not corrected all state correction orders from the prior survey. Deficiencies included failure to provide interconnected smoke alarms, incomplete fire safety and evacuation plans, lack of required training and drills, and failure to complete comprehensive resident assessments within required timeframes.
