Visit Reason
The inspection was a biennial survey to assess compliance with regulations governing the licensing and functioning of assisted housing programs, specifically Level IV Residential Care Facilities and Infection Prevention and Control.
Findings
The facility was found non-compliant in several sanitation and physical plant requirements, including missing thermometers in chest freezers, unlabeled and undated refrigerated food items, inadequate dishwasher rinse-water temperature, excessive hot water temperature in resident areas, and improper storage and labeling of poisonous and toxic materials.

Visit Reason
The inspection was a biennial survey to assess compliance with the Regulations Governing the Licensing and Functioning of Assisted Housing Programs for a Level IV PNMI Residential Care Facility.
Findings
The facility was found not in substantial compliance due to multiple deficiencies including an outdated therapeutic diet manual, unclean kitchen equipment, dishwasher rinse-water temperature below manufacturer specifications, poor maintenance and housekeeping in various areas, water temperatures exceeding 120°F in resident areas, and inadequate insect and rodent control due to an unscreened window used for ventilation.

Equipment used in food storage was not maintained in a clean and sanitary manner, including brown stains in freezers and debris buildup in refrigerator vents.
Dishwasher rinse-water temperature did not meet manufacturer specifications; cycles reached only 115°F instead of required minimum 120°F.
Facility showed lack of routine maintenance and housekeeping with multiple issues such as debris in fan vents, worn paint on toilet seat, missing cover on food storage receptacle, holes in walls, damaged caulking, rotting handrails, unsecured and broken windows, detached boards, cracked light cover, and damaged interior paint.
Water temperatures in resident areas exceeded 120°F, measuring 128°F and 131°F, but were adjusted to acceptable levels before exit.
Window used for ventilation was unscreened and unsecured, allowing gaps to outer air.
