The purpose of this visit was to conduct a compliance inspection at the facility.

Findings
The inspection identified multiple deficiencies related to workforce qualifications and training, including lack of current certification in emergency first aid and CPR for one staff member, missing tuberculosis screening and physical examination documentation, and missing required resident documentation such as signed Resident's Rights forms and National Sex Offender Registry search results.
