Inspection Report Summary for Danville Centre for Health and Rehabilitation

The most recent inspection on October 27, 2025, found the facility in compliance with no deficiencies identified. Earlier inspections included a January 24, 2025 abbreviated survey that cited deficiencies related to inadequate supervision and care planning after a resident eloped, which had resulted in an Immediate Jeopardy that was later removed following corrective actions. The main issues involved resident supervision and care planning to prevent elopement, with the facility addressing these through enhanced monitoring, staff training, and security improvements. Complaint investigations were mostly unsubstantiated, with the notable exception of the elopement-related findings in early 2025. The inspection history shows improvement, with the most recent survey indicating resolution of prior concerns.
