Inspection Reports for Delzen’s Place

The inspection was an unannounced required annual visit conducted using the CARE Inspection Tool to evaluate compliance with licensing requirements.

The facility was found to be in compliance with no deficiencies noted. The physical plant, infection control practices, medication records, emergency preparedness, and personnel files were all satisfactory.

The inspection was conducted as a pre-licensing visit for the purpose of evaluating the facility's readiness to operate as a Residential Care Facility for the Elderly (RCFE) with a capacity of 6 residents.

The facility met all necessary requirements including resident bedrooms, bathrooms, fire safety equipment, and operational appliances. Fire clearance was approved, and smoke and carbon monoxide detectors were operational. Chemicals and medications were properly secured. The final application approval will be issued by the Centralized Applications Bureau.

The visit was an initial licensing evaluation for Delzen's Place facility to verify the applicant and administrator's understanding of community care facility licensing laws and readiness for licensing.

The applicant and administrator participated in a virtual interview confirming their knowledge of licensing laws, facility operation, admission policies, staffing, emergency preparedness, complaints reporting, and pre-licensing readiness. Signed documentation and photo ID were obtained.
