Visit Reason
The inspection was a biennial survey to assess compliance with regulations governing the licensing and functioning of the assisted housing program at Dionne Commons, a PNMI Level IV Residential Care Facility.
Findings
The facility was found not in substantial compliance due to deficiencies in staff training, food storage practices, and general maintenance and housekeeping.
