Visit Reason
The inspection was a biennial survey to assess compliance with regulations governing the licensing and functioning of the assisted housing program at Dionne Commons, a PNMI Level IV Residential Care Facility.

Findings
The facility was found not in substantial compliance due to deficiencies in staff training, food storage practices, and general maintenance and housekeeping. Specific issues included one staff member lacking required certification, food items stored directly on the floor, and multiple maintenance and cleanliness problems throughout the facility.
