Visit Reason
The inspection was conducted as an annual review of Divine Manor Assisted Living Center to assess compliance with licensing, medication administration, safety, and resident care standards.
Findings
The inspection found multiple deficiencies including missing evacuation route postings, unavailable current licenses and inspection documents, incomplete quarterly fire drills, and numerous medication administration record (MAR) errors with missing staff signatures for multiple residents. Some physical exams and care plans were also not available for review.
