EAGLE CREST HEALTH CARE L.L.L.P underwent a comprehensive inspection on July 18, 2022, resulting in three state standard violations. Two of these deficiencies were classified under Health Code and one under Life Safety Code. The Health Code issues included the facility's failure to search the National Practitioner Data Bank (NAR) and Electronic Medical Record (EMR) annually, which was corrected on February 10, 2022.
