During a virtual tour of resident #1's room, it was found that the resident's call light was improperly positioned across the room and wrapped upward across the ceiling, and the call system did not activate when the call cord was pulled.

The inspection identified multiple deficiencies including lack of resident health care plans addressing treatment goals and effectiveness, failure to provide written copies of residents' rights, inaccessible call cords in resident rooms, improper storage of clean linens in the laundry room, unsecured laundry room with visible chemicals, and maintenance closet obstructed by paint cans and debris blocking access to electrical and maintenance panels.
