The evaluator cited a reporting‑requirements deficiency under CCR 87303(a) and required the facility to submit all required reports within the next eight days. The business office manager acknowledged receipt of the rights notice, and a Plan of Correction was set to address the documentation gap.
A third visit on July 28, 2022 is noted in the system, but the report content is incomplete.
