Inspection Report Summary for Elms Residential Home Care

Elms Residential Care was the subject of three unannounced complaint investigations in 2021. In July, evaluator Angel Ascencio found that housekeeping practices were insufficient, with residents reporting that rooms and bathrooms were not cleaned for weeks and that staff were often unable to maintain a clean environment due to staffing shortages. The investigation substantiated the allegation of poor facility cleanliness, citing non‑compliance with the California Code of Regulations § 87303(a) and requiring a corrective plan to establish a reliable cleaning schedule.

In October, evaluator Joann Rosales determined that the facility failed to assist a resident with prescribed medications, specifically Ondansetron PRN doses that were omitted during certain shifts. This failure was deemed a direct violation of § 87465(a)(5), which requires assistance with self‑administered medications. A corrective plan was issued, with the administrator agreeing to document staff training and ensure medication protocols are followed to protect resident health.

The November investigation addressed a separate allegation that staff had not supplied a resident’s COVID‑vaccination dates to the resident’s health‑care provider. After reviewing communications with the case manager and staff, the evaluator concluded that the claim was unsubstantiated. While the facility had knowledge of the vaccinations, it could not confirm dates at the time of the provider’s request, and no regulatory violation was identified.

Across the three reports, a recurring theme is gaps in routine care processes—cleaning and medication administration—often linked to staffing limitations or insufficient oversight. The July and October findings led to actionable corrective measures, whereas the November case highlighted an information gap that did not meet the threshold for a regulatory deficiency. Collectively, the investigations underscore the importance of robust staffing, clear communication, and consistent adherence to care protocols to maintain compliance and safeguard resident well‑being.
