In contrast, the complaints that a resident had wandered from the facility and that staff had neglected residents’ hygiene needs could not be verified; staff statements and resident interviews found no evidence of wandering or neglect, and no incident reports were available. The facility subsequently completed an in‑service training on medication documentation and provided documentation to the licensing agency, satisfying the required corrective action.
A second unannounced investigation conducted on April 12 2022 addressed allegations that residents were not receiving food on time, were served cold meals, that staff failed to follow illness‑prevention protocols, and that the facility harbored bed bugs. After reviewing meal schedules, interviewing staff and residents, and inspecting the kitchen, the evaluator found no evidence to support any of these claims. The evaluator also confirmed that the facility complied with COVID‑19 screening protocols and that a pest‑control invoice had been received, but there was no documentation of bed‑bug infestations. Consequently, all four allegations were determined to be unsubstantiated.
Across both investigations, a pattern emerges in which the facility’s documented processes for medication management were the sole area of substantiated deficiency. While staff and administrators consistently reported adherence to hygiene, meal service, and illness‑prevention standards, the licensing agency could not establish a preponderance of evidence for the remaining complaints. The corrective action plan issued for medication logging was completed, and no further violations were identified during the follow‑up. The reports indicate that Emerald Oaks has addressed the identified deficiency and that, at present, its operations comply with regulatory requirements in the areas investigated.
