Fairwinds – Woodward Park, a 740‑type assisted living facility in Fresno, was the subject of five unannounced licensing visits between March and August 2022. The March 25 inspection, focused on infection control, confirmed that the facility met all required standards: hand‑sanitizer stations were present, symptom screening signage was in place, and staff consistently wore face coverings. No deficiencies were cited and the evaluator noted the facility’s mitigation plan had been approved in February 2021.

A subsequent April 1 case‑management review yielded a similar outcome, with no deficiencies noted. However, a June 2 inspection uncovered a significant reporting lapse. The administrator had failed to submit Special Incident Reports for 12 residents and a staff member who tested positive for COVID‑19 within the previous month. Because these reports were not filed within the required seven‑day window, a citation under CCR 87211(a)(1)(D) was issued, and a plan of correction became due on June 16.

The June 22 follow‑up confirmed the plan of correction had not been submitted, resulting in a civil penalty of $600 ($100 per day for six days). By August 2, a new deficiency surfaced: the main kitchen air‑conditioning unit was turned off to prevent a water‑pipe leak, creating a potential health and safety risk. A deficiency under Section 87303, Maintenance and Operation, was cited with a new plan of correction due August 9. The evaluator noted that the HVAC system’s inactivity and exposed ceiling panels signaled ongoing maintenance issues that could compromise resident safety.

Across the inspections, a pattern emerges: early compliance with infection control and case‑management protocols contrasted with later lapses in COVID‑19 reporting, corrective action follow‑through, and facility maintenance. These findings highlight the need for strengthened administrative processes and timely corrective action to ensure continued compliance with California’s community care licensing requirements.
