Visit Reason
The Minnesota Department of Health conducted a full survey to evaluate and assess compliance with state licensing statutes for Family Choice Assisted Living LLC.
Findings
The licensee was found to be in substantial compliance but had several deficiencies including food service violations, lack of a complete emergency disaster preparedness plan, incomplete fire safety and evacuation plans and training, failure to provide required emergency relocation notice, and improper medication storage.
