Visit Reason
The Minnesota Department of Health conducted a full survey to evaluate and assess compliance with state licensing statutes for Family Tree Care Homes.
Findings
The licensee was found to be in substantial compliance but had several deficiencies including failure to document training and competencies for unlicensed personnel administering medications, incomplete emergency preparedness and fire safety plans, inadequate fire safety training, failure to maintain physical environment in good repair, and incomplete documentation and assessment of bed rail entrapment zones for residents.
