The inspection was an initial certification visit conducted to determine compliance with certification rules for an Assisted Living Program.
Findings
Multiple regulatory insufficiencies were cited including failure to complete required criminal history and abuse record checks for employees prior to hire, incomplete tenant health evaluations prior to and within 30 days of occupancy, and inadequate documentation of tenant medical information in nurses' notes.
