Visit Reason
The purpose of this visit was to conduct the compliance inspection of the First Senior Center.
Findings
The inspection identified multiple deficiencies including failure to obtain criminal background checks prior to employment, lack of work-related training on influenza and vaccination for staff, incomplete personnel records, missing physical examinations and tuberculosis screenings for staff, and failure to conduct required in-person interviews and tuberculosis screenings for several participants prior to admission.

Failed to provide work-related training on the nature of influenza and the role of vaccination to 4 of 5 sampled staff.

Failed to ensure all staff had received a physical exam by an authorized healthcare professional within twelve months prior to employment for 1 of 5 sampled staff (Staff B).
Failed to ensure all staff and volunteers were free of active tuberculosis with negative TB skin test or chest x-ray within twelve months prior to employment for 1 of 5 sampled staff (Staff B).
Failed to ensure prior to provision of services, the Director or designee had at least one in-person interview with the applicant and/or representative for 4 of 6 sampled participants.
Failed to ensure each participant was free of active tuberculosis based on negative TB skin test or chest x-ray within 12 months prior to admission for 3 of 6 sampled participants.
