Visit Reason
The purpose of this visit was to conduct the compliance inspection of the First Senior Center.

Findings
The inspection identified multiple deficiencies including failure to obtain criminal background checks prior to employment, lack of work-related training on influenza and vaccination for staff, incomplete personnel records, missing physical examinations and tuberculosis screenings for staff, and failure to conduct required in-person interviews and tuberculosis screenings for several participants prior to admission.
