Visit Reason
The Minnesota Department of Health conducted a survey to evaluate and assess compliance with state licensing statutes for an assisted living facility.
Findings
The facility was found in substantial compliance but had multiple level two violations including failure to prepare and serve food according to Minnesota Food Code, incomplete emergency preparedness plan, inadequate fire protection and physical environment maintenance, incomplete fire safety and evacuation plans and training, failure to comply with smoking laws and policies, and failure to complete timely resident reassessment related to smoking risks.
