The inspection was conducted in response to Complaint #NJ00168700 regarding compliance with New Jersey Administrative Code standards for licensure of dementia care homes.

The facility was found not in compliance with standards, specifically the failure of the facility administrator to ensure implementation of the policy on Resident Records for Resident #2. The service plan was not updated as required, and the surveyor observed deficiencies in documentation and care planning.

Complaint #NJ00168700 was substantiated based on observation, interview, and record review indicating noncompliance with licensing standards related to Resident #2's care and documentation.
