Visit Reason
The purpose of this visit was to conduct the compliance inspection with an onsite visit made to the facility on 2/7/24.

Findings
The facility failed to meet several workforce qualification and training requirements, including ensuring staff received at least 16 hours of training per year, tuberculosis screening and physical examinations prior to employment, and criminal fingerprint background checks. Additionally, the facility failed to maintain a medication assistance record (MAR) for one sampled resident.
