Visit Reason
The inspection was conducted as a behavioral health survey to assess the safety and appropriateness of the environment for adolescent consumers.
Findings
The survey found that the center did not fully implement programs in a safe and appropriate environment, noting unsecured outside doors and lack of awake staff supervision on weekend nights. A plan of correction was proposed to provide awake-night supervision during weekend shifts by July 1, 2004.
