Visit Reason
The inspection was conducted as a behavioral health survey to assess the safety and appropriateness of the environment for adolescent consumers.
Findings
The survey found that the center did not fully implement programs in a safe and appropriate environment, noting unsecured outside doors and lack of awake staff supervision on weekend nights. A plan of correction was proposed to provide awake-night supervision during weekend shifts by July 1, 2004.

The adolescent girls' bedrooms have outside doors without alarms or alert devices, and staff are not awake on weekend nights to monitor consumers, compromising safety.
