On 08/13/2025, a surveyor conducted an abbreviated survey at Golden Years Assisted Living Facility to assess compliance with regulatory standards.
Findings
Five deficiencies were identified, including one repeat violation related to failure to ensure employee screening for communicable diseases, failure to post grievance procedures, absence of exit and emergency exit diagrams, and deficiencies in conducting required fire and other emergency evacuation drills.
