Visit Reason
The inspection was conducted to assess compliance with the Regulations Governing the Licensing and Functioning of Assisted Housing Programs: Level III Residential Care Facilities, Part of 10-144, Chapter 113.

Findings
The facility was found not in substantial compliance due to deficiencies in resident rights regarding confidentiality and consent for release of information, failure to maintain routine maintenance and repair of wear and tear in a timely fashion, and improper storage of poisonous and toxic cleaning chemicals.

Resident records did not contain a current written consent to release information for Resident #1 and Resident #2’s Primary Care Physician.
Facility failed to show evidence of routine maintenance/repair of wear and tear in a timely fashion, including broken baseboard heater vents, rusty baseboard vent, rotting wood on deck rails, and chipping paint on decks.
Poisonous and toxic cleaning chemicals were not properly stored in Resident Bathroom #1 and Bathroom #2, including Lysol disinfectant and Febreze air freshener left out and not in use.
