Visit Reason
The investigation was initiated due to a complaint regarding a medication error where a direct care staff member administered liquid Ipratr-Albuterol orally instead of through a nebulizer to two residents.
Findings
The investigation confirmed that on 08/15/2025, direct care staff member Tessa Myers administered albuterol orally instead of via nebulizer to Resident A and Resident B. Ms. Myers was inadequately trained and administered medications independently after only two days of shadowing. Both residents were receiving hospice care and passed away within a week of the incident. The medication error was substantiated and violations of medication administration rules were established.
