Sep 8, 2025
Visit Reason
The investigation was initiated due to a complaint regarding a medication error where a direct care staff member administered liquid Ipratr-Albuterol orally instead of through a nebulizer to two residents.
Findings
The investigation confirmed that on 08/15/2025, direct care staff member Tessa Myers administered albuterol orally instead of via nebulizer to Resident A and Resident B. Ms. Myers was inadequately trained and administered medications independently after only two days of shadowing. Both residents were receiving hospice care and passed away within a week of the incident. The medication error was substantiated and violations of medication administration rules were established.
Complaint Details
The complaint was substantiated. The medication error involved improper administration of albuterol by a new staff member with inadequate training. The licensee designee was labeled as a perpetrator on the APS case for neglect due to allowing untrained staff to administer medications.
