Visit Reason
The Minnesota Department of Health conducted a full survey to evaluate and assess compliance with state licensing statutes for Guardian Care Homes Inc.
Findings
The licensee was found in substantial compliance but had several violations including falsification of personnel records, failure to maintain infection control and tuberculosis prevention programs, deficiencies in emergency preparedness and fire safety plans, physical environment maintenance issues, incomplete resident reassessments, and medication storage labeling issues.
