The inspection was conducted following a complaint received on 11/15/2024 regarding an incident on 11/8/2024 where Resident A eloped from the facility and was found injured outside.

The complaint was substantiated. Resident A eloped from the facility on 11/8/2024 between 4:30 AM and 5:15 AM. Employee #1 confirmed the event and that the facility had seven residents that night with only one staff member working. Resident A was found outside with injuries. Notifications were made to family and hospice. The investigation found deficiencies in supervision and documentation.
