Visit Reason
The inspection was conducted in response to a complaint alleging that there were no English-speaking staff in the home and concerns that staff were labeled as volunteers.
Findings
The investigation found that one staff member, Mr. Jairo Pernia, spoke very limited English and was unable to communicate effectively or handle emergency situations when working alone. The facility was otherwise clean, residents were well cared for, and other staff had sufficient English skills. Violations of direct care staff qualifications and training rules were established.
