Visit Reason
The investigation was initiated due to a complaint alleging that on 01/25/2024, Emergency Medical Services transported Resident A to the home but no staff could open the front door to allow entry.
Findings
The investigation found that Resident A was not attended to by staff when EMS arrived around midnight on 01/25/2024. EMS knocked on all doors and called the phone number posted on the front door multiple times with no answer, resulting in Resident A being returned to the hospital. The licensee designee's Google Voice number was posted on the door but did not receive calls from EMS. Staff denied sleeping on shift and no calls were received at the home number.
