A standard survey was conducted on 06/20/2025 for Heritage House of Portage to determine if the facility was in substantial compliance with Wisconsin Statutes and Administrative Code requirements for Residential Care Apartment Complexes (RCAC).

Tenant 1 did not receive adequate nursing services related to health monitoring, including running out of oxygen in the dining room due to staff not being trained to refill portable oxygen tanks, and missing a doctor's appointment due to lack of portable oxygen tanks. Observations included oxygen tubing wrapped around Tenant 1's legs and dragging on the floor, and staff confirmed lack of training on oxygen tank refilling.
